I clicked on new patient packet and it listed information about my child. I am filling it out as an adult patient. Is there another form?
Danielle – I am sorry you are having an issue. We have two separate new patient packets one for a child and another for an adult the adult packet asks how many children you have but should be primarily focused on the adult. Feel free to call the office if you are still struggling unfortunately it is too difficult to give you instructions otherwise.
